This is due to take place at Oaks Park High School on Tuesday 25th February.  

Please complete one consent form only for each child to ensure your child receives their vaccination. The link can be found in the attached letter above. 

The deadline for submitting your e-consent form is midnight on Tuesday 18th February.

Once the portal has closed, you won’t be able to complete the form online and will have to contact the immunisation team.
